List of documents for receiving diploma
Request for issuance of the student’s Diploma and Diploma supplement in English Language
Confirmation of receiving diploma
The clerance card will be available in the University's electronic system after logging in to the Virtual Dean's Office (after submitting a complete set of documents). Please check the status of your settlement with the University and settle all arrears.
There is no obligation to return your student ID.
The date of completion of studies (i.e. obtaining the title of Lekarz) is the date of the last credit or examination required by the curriculum.
Diploma payment
The graduate receives an original diploma together with a diploma supplement and two copies of each.
In the addition, at the student's request:
a copy of the diploma in a foreign language,
a copy of the diploma supplement in a foreign language.
At the request of the student , the university issues an additional copy of the diploma or a copy of the diploma supplement in a foreign language - a fee of PLN 20 is charged for each copy.
