On 1 January 2019, the Medical University of Białystok set up a partnership with StudioMcFIT, a company offering fitness courses and gym services, address: ul. Bohaterów Monte Cassino 10, Białystok.

The monthly cost of a personal card for unlimited access is PLN 35 and will be deducted in full from the employee’s monthly salary.
Employees interested in receiving a file a written statement of interest in the programme, with their consent to deduction of the card cost from their monthly salary (Appendix 1). They also have to submit a statement of consent to the processing of their personal data (Appendix 2). The deadline for the above submissions is 14 December 2018.
When the benefit threshold is exceeded, the Company Social Benefits Fund will be triggered; when the value of card payments exceeds PLN 200, this will happen in June 2019, and when PLN 300 is exceeded, it will be September 2019; or it may happen when an employee files a statement of withdrawal from the programme.
Moreover:
An employee can get a personal card for another person (Appendices 1A and 3), i.e. for their spouse or 15+ child, or for a third party. Every employee can only file an application for one such person. The cost of that person's card (PLN 35 monthly) will also be deducted from the employee's monthly salary.
Use of the service can be temporarily suspended, or the employee can withdraw from the programme altogether by giving a notice in writing by the 10th day of the month preceding the month of suspension or withdrawal (Appendix 4 or 5).
